绿帽社 considers accommodation requests for Emotional Support Animals (ESAs) and Service Animals in residence halls.

Emotional Support Animals (ESA) and Service Animals are not permitted on campus until the appropriate request has been reviewed and approved. Bringing unauthorized animals on campus may result in violations of the Student Code of Conduct.

Eligibility & Restrictions

鉁 Approved ESA's may include small, domesticated animals such as dogs (9+ months old), cats, rabbits, hamsters, birds, or fish.
鈿狅笍 Animals posing health or safety risks in communal living spaces may not be approved.

Steps to Apply for Accommodations

1. Complete and Submit

 

2. Submit Supporting Documentation

  • ESA Provider Form
  • Vaccination Records
  • Picture of the ESA/Service Animal
  • Roommate Agreement Form (if applicable)
    • If the student has a roommate, the roommate must consent to living with the ESA or Service Animal by signing a Roommate Agreement Form. This form will be sent directly to the roommate(s) once the request has been approved.

馃抢&苍产蝉辫;Note: Incomplete or insufficient documentation may delay the review process. Intake meetings may be rescheduled if submitted materials do not meet our documentation guidelines. Please review these requirements carefully before submitting your request. 

 

3. Application & Documetation Review

  • Once received, Disability Services will review the request and follow up with next steps, which typically include scheduling an intake meeting.
